Questions on the ETIAS form
The ETIAS authorization form contains specific questions about your identity, your trip and your personal history. Find out more about the main sections of the form on the official ETIAS website and how to prepare your answers.
To obtain your certificate, complete the ETIAS form online.
Questions about your identity
This section collects your personal information, such as :
- Surname, first name and gender, identical to your passport ;
- Date and place of birth ;
- Nationality as shown on your passport for Europe;

Passport details
As part of an ETIAS application, you will be asked to provide your passport details:
- Your passport number
- Passport issue and expiry dates
In the case of an ETIAS application for a minor, the information to be provided is exactly the same.
Questions about your trip to Europe
This part of the ETIAS questionnaire concerns the travel program within Europe. You are asked to fill in :
- Main destination country: The country where you will be staying the longest.
- Purpose of trip: tourism, business, transit, etc.

Health and safety information
These questions concern your medical and legal history:
- Serious medical history: Certain infectious diseases may be mentioned.
- Criminal record: Linked to serious offenses (terrorism, trafficking, etc.).
- Travel to conflict zones: Declare your recent stays.
Additional checks: You will also be asked to answer specific questions about any previous expulsions or visa refusals.
Final steps after validation
Once you have completed the form, you will be asked to pay your ETIAS online to finalize your application. There is no need to print out your ETIAS authorization before you set sail for Europe.
